My symptoms are as follows: Both testicles are swollen, and the swelling has gradually increased over several months. The epididymis and the upper part of the spermatic cord are the most affected areas. One testicle is longer than the other. The swelling and heaviness are worse in the morning. Occasional infections occur, with symptoms such as painful urination. I am concerned that the swelling may affect testicular function or fertility. Questions I would like your guidance on: What is the most likely condition causing these symptoms? Do I need specific tests such as a scrotal ultrasound with Doppler or urinalysis/STI screening?
