I have a patient who was diagnosed with HFReF, was prescribed empha 10mg, carvedilol 3.125mg, enalapril 5mg and aldactone 20mg. the patient is also hypertensive and type 2 diabetic. she is 65yo, the sympoms are coming back after 1 week of discharge, despite adhering to medicine, what might be the issue? other medications she's on includes metformin 500mg, glibenclimide 5mg and artovastatin 20mg
