Provisional diagnosis:

1) Gastroesophageal Reflux Disease (GERD)
2) Functional Dyspepsia
3) Anxiety/Panic Disorder with Hyperventilation Syndrome

Investigation:
1) ECG
2) Complete Blood Count (CBC)
3) Serum Electrolytes
4) Serum B12 and Vitamin D
5) Thyroid Profile (TSH, FT3, FT4)
6) H. pylori antigen/stool test or urea breath test
