Mam we had given mucinac 600tablets to him is that prevent from poisoning?
Giving Mucinac 600 (N-Acetylcysteine, NAC) was a good step, but it may not be enough to fully prevent liver damage, especially with repeated overdoses and alcohol use. Why? 1. Dose & Timing Matter – NAC is most effective when given within 8-10 hours of overdose. After that, its effectiveness decreases, but it can still help. 2. Repeated Overdoses – Your friend has taken large doses multiple times, increasing the risk of delayed liver failure even if there are no immediate symptoms. 3. Alcohol Increases Risk – Alcohol depletes liver enzymes, making acetaminophen even more toxic. What to Do Now? • Take him to the hospital immediately – He needs liver function tests (LFTs), INR, and blood tests to check for toxicity. • Doctors may need to give IV NAC – The oral dose you gave may not be enough. • Watch for Symptoms – Nausea, vomiting, abdominal pain, yellowing of skin/eyes (jaundice), confusion, or excessive tiredness require emergency care. This is serious and potentially life-threatening. Do not wait for symptoms to appear—get medical help now.

This is a serious case of paracetamol poisoning, which can be life-threatening. Your friend has repeatedly taken a toxic dose of paracetamol (Dolo 650) with alcohol, which significantly increases the risk of acute liver failure and other complications. Immediate Actions Required: Seek emergency medical help immediately – Liver function tests (LFT), PT/INR, and blood ammonia levels need to be checked. N-acetylcysteine (NAC) IV treatment may be necessary to prevent further liver damage. Monitor for symptoms of liver damage – If there are signs like abdominal pain, vomiting, yellowing of eyes or urine, confusion, or unconsciousness, urgent hospitalization is required. Alcohol makes paracetamol overdose more toxic, increasing the chances of severe liver injury, so ICU admission may be necessary in critical cases. Do not self medicate the patient, kindly bring this patient to Emergency department asap. Understanding the Cause of Repeated Overdose: Since your friend has overdosed multiple times, this could indicate intentional self-harm or an underlying mental health issue like depression, anxiety, or emotional distress. Identifying the root cause is crucial. A psychiatric evaluation is essential to assess suicidal thoughts and overall mental health. Emotional and social support is important—family and friends should provide a strong support system. Future overdose prevention is necessary—stress management and proper psychiatric treatment can help reduce the risk of recurrence. This is not just a medical emergency but also a serious mental health concern. Immediate medical and psychiatric intervention is necessary. Dr Aman Shaba Consultant Psychiatrist
