Doctors’ responses

Dear K. Narasimha Murthy, I understand that dealing with prostatomegaly (enlarged prostate), a prostatic cyst (8mm), and a history of right-side inguinal hernia surgery raises concerns about whether medication will be enough or if surgery is required. Understanding Your Condition: • Prostate Size: 40cc – This is moderately enlarged (normal size is 20-30cc). • Prostatic Cyst (8mm) – Small cysts in the prostate usually don’t need surgery unless they cause severe urinary issues. • Left Gross Varicocele – If asymptomatic, it may not require treatment. • Uroflowmetry (384/9/5) – Indicates moderate urinary obstruction (peak flow rate of 9 ml/sec is lower than normal). Is Surgery Needed? ✔ If symptoms are manageable with medication, surgery can be avoided for now. ✔ If symptoms worsen (frequent urination, weak stream, urine retention, repeated infections), then surgery (TURP or laser enucleation) may be needed. About Your Medication (Silodol-D8/0.5mg) • Silodosin (8mg) – Relaxes the prostate muscles to improve urine flow. • Dutasteride (0.5mg) – Shrinks the prostate over time. How long should you take it? ✅ Usually taken long-term (at least 6 months to 1 year for best results). ✅ Regular check-ups (every 6 months with a urologist) are important to monitor progress. Next Steps & Precautions: 🔹 Repeat Uroflowmetry & PSA Test – Every 6 months to check prostate health. 🔹 Abdominal Ultrasound – To monitor cyst & residual urine. 🔹 If symptoms worsen despite medication – Consider surgical options (TURP, HoLEP, or laser prostatectomy). Since you also have diabetes & hypertension, surgery (if needed) must be planned carefully. For now, continue medication, monitor symptoms, and follow up with your doctor every 6 months. Let me know if you need further clarification. Take care!

Left-sided varicocele at your age can often be managed without surgery if the symptoms are mild. Since you’ve had a right-side inguinal hernia surgery before, it’s important to monitor the condition carefully. The tablet Silidol-D8 (Tamsulosin 0.4 mg + Dutasteride 0.5 mg) you are taking is typically prescribed for benign prostatic hyperplasia (BPH) to improve urine flow and reduce prostate enlargement. You may need to continue taking this medication long-term as it helps manage urinary symptoms, but your doctor will decide the duration based on regular follow-ups. It is recommended to undergo routine prostate check-ups and urine flow tests to monitor your condition. If your varicocele causes persistent pain, discomfort, or fertility issues, or if it significantly impacts your quality of life, surgery may be considered. Varicocelectomy or embolization are the common surgical options. However, at your age, surgery is usually recommended only if symptoms are severe, as the risks may outweigh the benefits. To prevent worsening, wear supportive underwear, avoid heavy lifting, and engage in light exercises to promote circulation. Stay hydrated and follow up with your urologist regularly to assess the need for further treatment. Let me know if you need more details or assistance.
