Doctors’ responses

Your chronic constipation, rectal ulcer, and GERD, combined with uncontrolled diabetes and fatty liver, indicate a complex condition that requires a comprehensive treatment plan. Since your constipation is severe, leading to rectal bleeding and ulcers, it’s essential to focus on bowel regulation. Along with stool softeners, add psyllium husk (isabgol) before bedtime and magnesium supplements to promote regular bowel movements. Eat a fiber-rich diet with vegetables, fruits, and whole grains while drinking plenty of water (2.5-3 liters daily) to soften stools. You can also take probiotics to improve gut health. For rectal ulcers, apply lidocaine gel or hydrocortisone ointment to reduce pain and inflammation, and consider sitz baths (warm water with Epsom salt) twice daily to promote healing. Your fatty liver and GERD require dietary and lifestyle changes. To reduce liver inflammation, limit refined carbs, sugary foods, and fatty meals, and eat a low-fat, high-fiber diet. Include green leafy vegetables, lean protein, and healthy fats like olive oil. For GERD, avoid spicy, acidic, and fried foods. Eat smaller meals, avoid late-night eating, and sleep with your head elevated. Take proton pump inhibitors (PPIs) or H2 blockers (as prescribed) to reduce acid reflux. Managing your diabetes is also critical—monitor your blood sugar regularly and consult your doctor for medication adjustments if needed. For your insomnia and weakness, focus on improving sleep hygiene. Avoid caffeine, alcohol, and screens before bed. Create a consistent sleep schedule and try relaxation techniques, such as deep breathing or meditation, before sleeping. If insomnia persists, consult your doctor for melatonin supplements or low-dose sleep aids. Since you’ve been hospitalized multiple times, consider visiting a gastroenterologist and hepatologist for a thorough evaluation, including imaging and endoscopy, to assess the severity of your rectal ulcer and fatty liver
