Thanks for sharing that—it sounds uncomfortable and probably a bit worrying, especially since it's an old burn scar that's now acting up. **Patient Case Summary **Name:** ________ Rahman **Age:** 20 years **Sex:** Female **Presenting Complaint:** Reactivation of symptoms over an old burn scar near the ear, associated with itching, redness, and recent onset of swelling for the past 2 days. **History of Present Illness:** The patient reports a childhood burn injury near the ear (exact degree unknown, but healed with scarring). For the past 2 days, the area has become red, itchy, and slightly swollen. There is no history of recent trauma, insect bite, or direct contact with irritants. No discharge or pus noted so far. The patient consulted a general physician and was prescribed a topical ointment and oral medication (names can be inserted if available). Despite medication use, the patient has noted **increasing swelling** and **persistent symptoms**. **Past Medical History:** - Childhood burn injury near left/right ear (healed with scar) - Nabothian cyst – resolved - History of irregular menstruation (ongoing concern) - No known drug allergies **Family History:** Non-contributory **Medications:** [List the ointment and oral medications if known] **Allergies:** No known drug allergies (NKDA) **Other Notable History:** Irregular menstruation for past 6 months, reduced bleeding days (1–2 days) **Review of Systems:** - No fever - No general malaise - No lymphadenopathy noted by patient - No recent systemic infection or cold **Provisional Diagnosis:** - Reactivated inflammation or infection over old burn scar (possible **scar dermatitis**, **contact dermatitis**, or **localized cellulitis**) - Differential: **Keloid/hypertrophic scar flare-up**, **folliculitis** **Recommended Workup / Next Steps:** 1. Local examination of scar (check for signs of infection: warmth, tenderness, discharge) 2. If infection suspected: - CBC, CRP (to check for inflammation) - Consider starting oral antibiotics if not already done 3. Dermatology referral for: - Scar evaluation (keloid vs. hypertrophic) - Allergy test (if contact dermatitis suspected) - Possible topical steroid/non-steroidal creams if inflammation is non-infectious **Plan:** - Continue/modify current topical treatment as per findings - Cold compress (avoid ice directly on skin) - Avoid scratching or applying cosmetic products - Monitor for signs of progression (fever, pus, pain increase) If you'd like, let me know the name of the ointment and pills prescribed, and I can insert them into this format as well.
