Doctors’ responses

Hi Mahalakshmi, I understand how frustrating and uncomfortable this must be for you. Since you’ve been dealing with itching for two months and have diabetes, the infection might be recurring due to uncontrolled sugar levels. Additionally, chronic constipation and your fissure seem to be adding to your discomfort. For the Itching (Possibly Fungal Infection): • Diabetes Control: It’s essential to manage your sugar levels, as high glucose can worsen itching and increase the risk of fungal infections. • Hygiene Tips: • Wash with plain water only and avoid any harsh soaps or vaginal washes. • Keep the area dry, and wear loose, breathable cotton underwear. • Continue Candid Cream: If it provided some relief, you can keep using it, but if the itching persists, oral antifungal medication may be necessary. For the Fissure and Constipation: • Lactulose Syrup: Continue taking it but consider adjusting the dose (with your doctor’s advice) if it isn’t softening your stool enough. • Diet Tips: • Increase fiber-rich foods like vegetables, whole grains, and fruits. • Drink at least 2-3 liters of water per day. • Avoid spicy and oily foods that can irritate your fissure. • Sitz Bath: Sit in warm water for 10-15 minutes twice a day to reduce pain and help the fissure heal. Next Steps: Since uncontrolled diabetes is likely contributing to both your itching and fissure, it’s important to work on stabilizing your blood sugar. Please consult your doctor for further treatment if the itching or constipation doesn’t improve. Take care, and I’m here if you have more questions!

The persistent itching in your private area is likely due to a yeast infection or diabetic vulvovaginitis, which is common in people with uncontrolled diabetes. Since you’ve had the issue for two months, you may need oral antifungal medication (like fluconazole) along with a topical antifungal cream (clotrimazole or miconazole) for 7-14 days. To soothe the itching, you can use calamine lotion or hydrocortisone cream temporarily. Maintain good hygiene by wearing cotton underwear, keeping the area dry, and avoiding scented soaps or washes. Since recurring infections can be a sign of poorly controlled blood sugar, managing your diabetes is essential. For your chronic fissure and constipation, continue using lactulose syrup but consider adding psyllium husk (isabgol) before bedtime to improve bowel movements. Eat a fiber-rich diet with vegetables, fruits, and whole grains, and drink plenty of water to keep stools soft. Taking sitz baths (warm water with Epsom salt) twice daily can help reduce pain and promote healing. You can also apply lidocaine gel or nitroglycerin ointment for pain relief. Avoid straining during bowel movements to prevent worsening the fissure. To better manage your uncontrolled diabetes, focus on dietary changes and regular monitoring. Limit refined carbs and sugary foods while including lean proteins, healthy fats, and fiber-rich foods. Stay hydrated and engage in regular physical activity to improve blood sugar levels. If your symptoms persist or worsen, consult a gynecologist for the vaginal infection and a proctologist or gastroenterologist for the fissure. You may also need to see your diabetologist for medication adjustments to achieve better glucose control.

Hello mahalakshmi ji. As you are a known diabetic with uncontrolled level, first advice would be to control your level as higher the level more chances of infection and fungal issues that would increase itching And as you have fissure that is also the cause of repeated action,as I would suggest you to understand fissure repair surgery that would help to prevent repeated infection after your hb1ac level are under 6.5 Third advice would be regular use of vwash for vaginal hygiene For dietary changes I would suggest to avoid sweet, rice, carb rich food instead of that you should eat ragi and high fibre diet with got amount of water. Surgery for fissure repair will help u with most of problems
