**Patient Name**: Pankaj Kumar **Age**: 22 years **Date**: 12-04-2025 **Presenting Complaints**: Pankaj reports a chronic history (3 years) of gastrointestinal discomfort including **excessive gas**, **frequent burping**, **poor digestion**, **bloating**, **abdominal pain and burning sensation**, **constipation**, and **unintentional weight loss**, which is contributing to **weakness and fatigue**. He has a past history of **gastritis** and was previously diagnosed with **H. pylori infection**. --- **Clinical Impression**: The symptoms suggest chronic **gastritis**, possibly related to **persistent or untreated H. pylori infection**, compounded by functional dyspepsia and **slow gut motility**. Constipation and gas can worsen acid production and bloating, creating a vicious cycle. Weight loss and weakness are concerning signs, often linked to poor nutrient absorption. --- ### **Treatment Plan**: 1. **Medical Treatment** (if H. pylori not treated or partially treated): - **H. pylori eradication therapy (as per current guidelines)** — may include a **triple therapy**: - **Tab. Pantoprazole 40 mg** – twice daily, before meals - **Cap. Amoxicillin 1000 mg** – twice daily, after meals - **Tab. Clarithromycin 500 mg** – twice daily, after meals - *Duration: 14 days (only under doctor supervision)* If already treated, continue with maintenance: - **Tab. Pantoprazole 40 mg once daily** (empty stomach) for 4–6 weeks - **Tab. Digene / Sucralfate syrup** – 2 tsp before meals for gastric protection 2. **Digestive Support**: - **Cap. Unienzyme or Vizylac** – once daily after main meal to improve digestion - **Probiotic** (e.g. Sporlac-DS) – once daily for 2–3 weeks to restore gut flora 3. **Constipation Management**: - **Isabgol (Psyllium husk)** – 1 spoon with warm water at bedtime - Increase **fiber intake** (fruits like papaya, guava, soaked raisins; vegetables like spinach, bottle gourd) - Warm water early in the morning and regular bowel timing to improve clearance 4. **Dietary Modifications**: - Avoid: **Spicy, oily, fried food**, tea/coffee on empty stomach, and carbonated drinks - Eat: Small frequent meals; bland diet with soft cooked vegetables, curd, rice, dal, oats - Include: **Coconut water**, **buttermilk**, **banana**, and **light khichdi** to soothe the stomach - Do not skip breakfast; try to eat at fixed times 5. **Lifestyle Advice**: - Avoid lying down immediately after meals - Elevate head during sleep if there is burning - Brisk walk or light movement after meals to aid digestion - Practice **yoga asanas** like *Pawanmuktasana*, *Vajrasana after meals* --- ### **Follow-Up Plan**: - Reassess after 2–3 weeks for symptom relief - If symptoms persist, further tests like **Urea Breath Test**, **Endoscopy**, **Stool for occult blood**, and **USG abdomen** may be advised - Maintain hydration, nutrition, and regular follow-up **Note**: Weight loss and chronic acidity must be monitored closely. Continue treatment and avoid OTC strong laxatives unless prescribed. Recovery is gradual but possible with consistent care. Advice : Do stool antigen test for H pylori Dr Shayeque Reza MD 9800280276

Is it reversible or not , iam having this problem from last five months and getting vomiting frequently and having GGt - 995, total bilirubin - 4.8, fasting sugar 180 BP- 160 hba1c-6.8, shot-180, frequently suffering with fever

Hello Mr. Mohan Rao. Your lab parameters indicate that you might be consuming higher than usual amount of alcohol , which could explain all the signs and symptoms of liver dysfunction. Correct me if I am wrong. Alcohol addiction is a real psychiatric disorder and needs proper treatment and management so as to prevent further psychiatric and medical complications. Management includes 1) Using anti-craving agents to help with reducing craving for alcohol 2) Psychotherapy- Motivation enhancement therapy and Relapse prevention therapy 3) Lifestyle modifications and diet modifications to address diabetes Kindly consult the nearest psychiatrist/ endocrinologist for further assessment and management. Regards, Dr. Charan Kumar, MD Psychiatry (NIMHANS).
